This is a re-validation service only for a pre-existing Medicare enrollment.

This re-validation service is for the healthcare provider who is:

• The Sole Owner of a corporation or organization (LLC, SMLLC, PC…) and will be the only provider enrolled with the organization/corporation at this time.

• This service includes registration for a new Type 2 NPI Number (for an Organization)

If you are a Sole Proprietor, or a single provider enrolling in a practice that has itself already been enrolled, you will need our Single Provider: Medicare (PECOS) Enrollment‍ ‍re-validation service.

Fees: Individual physicians, non-physician practitioners, and certain changes of information that do not involve a change of ownership or new location are typically exempt from Medicare enrollment fees. Some organization/corporations may be subject to a $750 CMS/Medicare Enrollment Fee. Your Credentialing Specialist will confirm this with you prior to application. The price for this service does not include any mandatory service fees required by CMS/Medicare to enroll.
