A one-time service that provides review and preparation of credentials + facilitation of one credentialing application for employment, licensing or privileges for a single provider.
All credentials required for the specific application (credential documents, DataSpring/CAQH Provider Profile, NPPES, FSMB, etc.) are reviewed for any of the following issues and corrections/updates made.
Facilitation of the application is also included and follows the review and updates to the credentials that will be included.
Service only includes credentials service and application preparation and does not include any mandatory application submission fees. Provider will be responsible for paying all transcript and application fees as required.
If the employer, licensing board/agency or medical staff committee mandate that the provider submit the application online by themselves, Credentials 360 will assist in all other preparation for the provider’s complete application up to the point of digital submission.
